You must extract both mp5zknivez.bsp, mp5zknivez.res mp5zknivez.nav and mp5zknivez.txt to:

C:\Program Files\Valve\Steam\SteamApps\\counter-strike source\maps

If you are hosting this map on a server please make sure all Four first are installed correctly.
